A medical campaign for eye surgery during the month of June 2022

SAMS organization is preparing to carry out a medical campaign during the month of June of the year 2022, during which eye surgery will be performed (opening the tear duct, surgically unblocking it, and beautifying the eyelids). The following questionnaire is to record cases from Syrian patients residing in Jordan and Jordanians in need.
This is to be studied by the Syrian American Medical Association team in the Jordan office, and then presented to the specialized doctors, and after the association completes all the necessary preparations, procedures and approvals, only the patients who were selected by the doctors are contacted.
