Psychological support case manager

Roles and responsibilities:
1. Identification of persons of concern who need support and a comprehensive psychosocial assessment of persons of concern referred or identified.
2. Identify the psychological problem(s) and develop a plan of action/care jointly with the persons of concern.
3. Basic psychosocial support is adequately provided for individual and group interventions at the level of non-specialized focused support according to the psychosocial intervention hierarchy.
4. Undertake and appropriately document home and field visits when needed and as appropriate for the context.
5. Referring cases to specific services (internal and external) to meet their needs, according to a jointly developed care plan.
6. Develop a map of services in the area covered by the center to ensure effective referrals, and periodically update the referrals map on a quarterly basis as a minimum.
7. Work closely and coordinate with staff in the workplace as needed, as well as with members of the case management team (CP and GBV case managers)
